Auf der internistischen Station betreuen Sie heute Frau Keller. Die Patientin ist sturzgefährdet und kann nicht alleine zur gehen. Sie melden sich an ihrem Bett an, erklären ruhig die nächsten Schritte und bereiten eine und ein vor. On the internal medicine ward you are caring for Mrs Keller today. The patient is at risk of falling and cannot go to the toilet on her own. You introduce yourself at her bedside, calmly explain the next steps and prepare a bedpan and a urinal. Before you begin, you put on disposable gloves and place a protective sheet under the patient’s buttocks to keep the bed clean. After urination you help Mrs Keller with wiping and gently pat the genital area dry. Then you dispose of urine and stool immediately via the sluice/drain, clean the bedpan thoroughly and disinfect it. This prevents unpleasant smells and helps reduce the risk of infection.

For nursing documentation you observe carefully: the urine is dark yellow today and the volume is rather low. You ask the patient whether she has drunk enough and recommend more water or tea. During bowel movements you notice the stool is very hard. Mrs Keller reports she has been constipated for three days. You record this in the nursing report and inform the doctor on duty. Abnormalities such as blood in the urine, severe diarrhoea or a very strong odour should always be documented precisely and reported promptly. This helps to identify complications early.

Thank you for your message about Mr. Keller.

I will take over the observations on the late shift. I will check urine volume and color at each toileting and ask him about any pain or increased pressure in the lower abdomen. I will also document whether and when a bowel movement occurs and the stool consistency.

If the urine volume remains low or new symptoms appear, I will inform the on-duty physician and record everything in the nursing chart.
