The palliative care service at the municipal hospital supports seriously ill people in the final phase of life. The team talks with patients and their families about wishes and fears — for example about pain, religious rituals, or the atmosphere in the room. The aim is to reduce pain and improve quality of life.

During counselling sessions, the nurse also explains documents such as advance directives and health care powers of attorney. This allows the patient to state which treatments they do or do not want. Relatives receive information and comfort. After a patient’s death, the family may receive a condolence card or a short farewell message. The staff express their sympathy and respect cultural differences and personal decisions.

First I will inform the responsible physician and discuss the further palliative care plan with them. We will then review how to improve his pain management, for example by giving pain medication at more regular intervals.
