Bitte beachten: Am Dienstag findet um 13 Uhr im Besprechungsraum die statt. Thema ist die Entlassplanung für Patientinnen und Patienten nach einer Operation. Bitte bringen Sie aktuelle Informationen aus Ihrer mit (Mobilität, Medikamente, Pflegebedarf, soziale Situation).

Please note: On Tuesday at 1:00 p.m. a case conference will take place in the meeting room. The topic is discharge planning for patients after surgery. Please bring up-to-date information from your area of responsibility (mobility, medications, care needs, social situation).

Attending will be: the consultant, the registered nurse, the physiotherapist, the social worker and, if needed, the pharmacist. The ward manager will chair the meeting. The aim is to coordinate and reach a joint decision at the end. If you cannot attend, hand your points in advance to the responsible nurse during handover.

Stationsleiter Herr Krüger: Show Frau Yilmaz, kurze Übergabe: Herr Meier in Zimmer 12 hat heute Visite, der Facharzt kommt um zehn.

Pflegefachfrau Frau Yilmaz: Show Okay. Ich bin aber unsicher: Ist für die Medikation der Allgemeinarzt zuständig oder der Facharzt?

Stationsleiter Herr Krüger: Show Der Facharzt trifft die Entscheidung, und die Pflegekraft gibt die Medikamente; bei Unklarheiten rufen wir den Apotheker an.
