Frau Novak ist 68 Jahre alt und hatte vor zwei Wochen einen . Sie liegt jetzt auf einer neurologischen Station in einem Krankenhaus in Deutschland.
Im Morgenbericht spricht die Pflegefachkraft mit der Physiotherapeutin. Im steht: rechte Seite teilweise gelähmt, wenig Gefühl im rechten Arm, unsicher beim Stehen. Die ist links normal, rechts schwach. Frau Novak hat manchmal Schmerzen im Schulterbereich.
Die Physiotherapeutin erklärt den : Täglich kurze , Training im Bett, später Sitzen an der Bettkante und Transfer in den . Ziel ist: bessere , sichere Transfers und weniger Schmerzen. In einer Woche gibt es eine mit Arzt, Pflege und Therapie.
Am Nachmittag übt die Pflegefachkraft mit Frau Novak das Umlagern im Bett. Sie erklärt langsam jeden Schritt und benutzt ein Gleitlaken. Dann hilft sie beim Transfer in den Rollstuhl. Sie erinnert Frau Novak an die Bremse am Rollstuhl und an die richtige Position der Füße.
Am Ende fragt die Pflegefachkraft: „Wie stark sind die Schmerzen, von 0 bis 10?“ Frau Novak sagt: „Heute sind die Schmerzen mittel, vielleicht 4.“ Die Pflegekraft schreibt das in den Pflegebericht und plant mit dem Team die nächste Therapieeinheit.Mrs Novak is 68 years old and had a stroke two weeks ago. She is currently on a neurological ward in a hospital in Germany.
In the morning report the nurse talks with the physiotherapist. The nursing note states: right side partially paralyzed, reduced sensation in the right arm, unsteady when standing. Muscle strength is normal on the left and weak on the right. Mrs Novak sometimes has pain in the shoulder area.
The physiotherapist explains the therapy plan: short physiotherapy sessions every day, bed exercises, later sitting on the edge of the bed and transfer to the wheelchair. The goals are improved mobility, safe transfers and less pain. In one week there will be an evaluation with the doctor, nursing staff and therapy team.
In the afternoon the nurse practices repositioning in bed with Mrs Novak. She explains each step slowly and uses a slide sheet. Then she helps with the transfer into the wheelchair. She reminds Mrs Novak to use the wheelchair brake and to place her feet correctly.
At the end the nurse asks, “How strong is the pain, from 0 to 10?” Mrs Novak replies, “Today the pain is moderate, maybe a 4.” The nurse records this in the nursing note and plans the next therapy session with the team.

Die Mobilität ist noch eingeschränkt. Sie fährt im Moment nur im Rollstuhl. Mit dem Gehstock kann sie noch nicht sicher gehen. Zur Toilette braucht sie immer eine Pflegekraft.
Beim Aufstehen braucht sie viel Hilfe. Sie kann sich im Bett langsam selbst umdrehen, aber sie ist noch unsicher. Beim Transfer vom Bett in den Stuhl muss ich sie stützen und absichern.
Frau Novak hat oft Schmerzen im linken Bein. Die Schmerzen sind meistens bei 4 bis 5 auf der Schmerzskala.
Für den Reha-Plan finde ich wichtig, dass wir weiter das sichere Aufstehen und Gehen üben.

Her mobility is still limited. At the moment she only moves around in a wheelchair. She cannot walk safely with a walking stick yet. She always needs a nurse to go to the toilet.
She requires a lot of assistance when standing up. She can slowly turn over in bed by herself, but she is still unsteady. During the transfer from bed to chair I need to support and secure her.
Mrs. Novak often has pain in her left leg. The pain is usually rated 4 to 5 on the pain scale.
For the rehab plan I think it is important that we continue practising safe standing up and walking.
