Bij het maken van een recept zijn duidelijke regels belangrijk. Je schrijft nauwkeurige instructies voor het gebruik van het medicijn, met de dosis per keer en de maximale dosis per dag. Het adres van de patiënt moet volledig kloppen en het moet duidelijk zijn waar het recept naartoe gaat. Op papier gebruik je een volledige handtekening, geen paraaf. Op één receptvel staat meestal maar één middel.

Het is belangrijk dat nauwkeurige instructies voor het gebruik van het geneesmiddel worden beschreven. (It is important that accurate instructions for the use of the medicine are described.)
Hierbij moeten zowel de dosis per keer als de maximale dosering per vierentwintig uur duidelijk vermeld worden. (Both the dose per time and the maximum dosage per twenty-four hours must be clearly stated.)
In plaats van een paraaf dient altijd een volledige handtekening geplaatst te worden. (Instead of initials, a full signature should always be provided.)
Vaak worden een paraaf en een handtekening als hetzelfde gezien, maar een paraaf is meestal te kort en juridisch niet geldig voor opiaten. (Initials and a signature are often seen as the same, but initials are usually too short and not legally valid for opiates.)
Het adres van de patiënt moet volledig en correct ingevuld zijn. (The patient's address must be completed fully and correctly.)
Het moet altijd duidelijk zijn waar het recept of de geneesmiddelen naartoe gaan. (It must always be clear where the prescription or the medicines are going.)
Per receptvel mag slechts één geneesmiddel worden voorgeschreven. (Only one medicine may be prescribed per prescription form.)
In het geval van een opiaat dat samen met bijvoorbeeld een laxans wordt gegeven, moeten twee aparte recepten worden gebruikt. (In the case of an opiate that is given together with, for example, a laxative, two separate prescriptions must be used.)
Tegenwoordig gaat het voorschrijven vaak digitaal, maar deze regels gelden nog steeds voor papieren recepten. (Nowadays prescribing is often done digitally, but these rules still apply to paper prescriptions.)
Een recept bevat bovenaan de gegevens van de voorschrijver, gevolgd door de R, de DA en de S, met daaronder de handtekening of paraaf, en helemaal onderaan de naam, geboortedatum en het adres van de patiënt. (A prescription contains the prescriber's details at the top, followed by the R, the DA and the S, with the signature or initials below that, and at the very bottom the patient's name, date of birth and address.)

Writing a medical prescription in the Netherlands
1. Assistent: Goedemorgen dokter, vandaag leg ik u uit hoe u in Nederland een medisch recept opstelt. (Good morning doctor, today I will explain to you how to write a medical prescription in the Netherlands.)
2. Tandarts: Perfect, dank u. Ik wil zeker weten dat ik alles goed doe. (Perfect, thank you. I want to make sure that I do everything correctly.)
3. Assistent: Eerst schrijft u linksboven uw achternaam en voornaam, als tandarts. (First, at the top left you write your last name and first name, as a dentist.)
4. Tandarts: Oké. En wat moet ik rechtsboven zetten? (Okay. And what should I put at the top right?)
5. Assistent: Rechtsboven schrijft u de datum, de naam van de kliniek, het adres van de praktijk, de postcode en de plaats. (At the top right you write the date, the name of the clinic, the practice address, the postal code and the city.)

7. Assistent: Onderaan zet u de achternaam en voornaam van de patiënt en de leeftijd. Bij een kind vermeldt u ook het gewicht in kilo. (At the bottom you put the patient's last name and first name and the age. For a child, you also mention the weight in kilos.)
8. Tandarts: En hoe schrijf ik de medicijnen op? (And how do I write the medications down?)
