Writing a medical prescription in the Netherlands

2. Tandarts: Perfect, dank u. Ik wil er zeker van zijn dat ik alles correct doe. (Perfect, thank you. I want to make sure I do everything correctly.)
3. Assistent: Eerst schrijft u linksboven uw achternaam en voornaam, met daaronder 'tandarts' of 'tandarts-chirurg'. (First, write your last name and first name in the top left, and beneath that put 'dentist' or 'dental surgeon'.)
