I usually say: "I first explain what we’re going to do and what it will roughly cost. If that’s okay with you, we’ll only start after that." If there is a surcharge, I explain why it’s necessary (e.g. extra time or special care) and that the surcharge applies per appointment.
Yes, I always note in the record what information I provided, that the patient agreed, and whether there were any questions.
