Message du Conseil départemental de l'Ordre : pour commencer votre inscription, envoyez la de diplôme, une pièce d'identité et un judiciaire récent. Joignez aussi une sur l'honneur et votre assurance responsabilité civile professionnelle ( ).
Si vous êtes chirurgien-dentiste salarié, ajoutez votre contrat ( ou ) et le nom de la structure. Si vous êtes libéral, indiquez votre statut. Could you specify the complete list of documents to send to validate my registration on the Order’s Register? I already have my ID document and the degree recognition. For the criminal record certificate, which document do you accept (bulletin no. 3)? And is the RCP certificate mandatory for an employee in a clinic?
Can I send the documents by email in PDF format?
