This is a registration form for a Spanish course for foreigners in Madrid. The course runs in the afternoon, Monday to Thursday. The cost is 150 euros per month. On the form, the student writes their name, country and phone number. They also indicate their Spanish level and provide an email address.

At the end of the form there are questions: “Why do you want to study Spanish?”, “How many hours do you want to study per week?” and “What time do you prefer the class?”. The student answers the questions and signs the document. Afterwards, they turn the form in at the school office or send it by email.
