You don’t need to explain everything, but you should say whether the plan is realistic and when you would like to start it.
They also ask whether anyone would like to request a short sabbatical (1 to 3 months) or devote time to a personal project, such as a course, volunteer work, or a trip. They recommend thinking about the budget and about returning to work, so that the end of the plan is clearly defined.
