Door de ziekte van zijn moeder kan Tom het werk en de zorg voor haar niet meer combineren. Daarom neemt hij contact op met het van de gemeente. Een medewerker plant een huisbezoek. He therefore contacts the municipality's Wmo desk. An employee arranges a home visit. During this meeting an assessment committee determines which support is needed: household help, accompaniment to appointments, or a few hours of respite care per week so Tom can take a break. He then receives a letter with the assessment and a proposal for a Wmo provision.
The letter states that a care coordinator will organize the care together with Tom. They discuss which tasks the informal caregiver will continue to do and which tasks home care will take over. They also draw up a care agreement stating how many hours of care are provided per week and how often they will evaluate together. Tom reads that he may be entitled to care allowance and that he must register with the chosen organisation for day care for his mother within two weeks.
