Reality: it did not happen / it went differently.

The basic formula (2 parts)

Als introduces a subordinate clause → the verb goes to the end in that clause.
The condition uses voltooid verleden tijd: had + voltooid deelwoord.

Word order you should check

Als-clause: “Als ik dat geweten had, …” (the had is at the end)
Main clause after a comma: the verb usually comes first.
- “Als hij zich beter had voorbereid, had hij de baan gekregen.”
- “Als ik dat geweten had, dan was ik gebleven …”

Which verb in the consequence: had or was?

The consequence is often also in the past perfect, but the auxiliary depends on the verb.

Most verbs: had + past participle
Verbs of movement/change of state (e.g., gaan, komen, blijven, worden): often was / waren + past participle

What does dan do?

Dan is optional.
It adds extra emphasis: “in that case”.
Use it especially when you want to sound clear or firm.

Pick the past fact (what really happened):
Ik las de jobomschrijving niet goed.
Make the Als-clause with past perfect:
Als ik de jobomschrijving beter had gelezen, …
Add the consequence (also in past context):
… had ik minder fouten gemaakt.
Optional: add dan for emphasis.

Exceptions!

You can also use dan for extra emphasis in the result part of the sentence.
